MRI report 10/18/11

MRI: abdomen without and with intravenous contrast

History: Evaluates for liver lesion seen on recent PET CT scan.

Comparison: PET CT scan, 9/20/2011, CT abdomen 12/10/2010

Technique: Multiplanar multisequence MRI of the abdomen performed with and without intravenous contrast, 20 ml. Magnevist contrast administered.

Findings:
Within the interior-inferior right hepatic lobe, there is a cystic area measuring 2.6 x 3.3 cm with minimal peripherel enchancement. This is in area of previous noted metastasis seen on prior CT scan and likely represents post treatment change. Within the posterior aspect of the right hepatic lobe seen best on series 8 and series 10 image 18, there is a 1.2 cm diameter lesion which demonstrates delayed enhancement which has increased in size from 8 mm on prior CT scan and is suspicious for metastasis. Within the posterior medial aspect of the right hepatic lobe best demonstrated on series 8 image 12 through 14, there is an ill-defined enhancing lesion measuring approximately 2.0 x 2.5 cm which corresponds to the area of increased metabolic activity seen on PET scan. This is suggestive of metastic disease.
Patient is status post cholecystectomy. The spleen, pancreas, and adrenal glands are unremarkable.

There are bilateral renal cysts measuring up to 2.1 cm in diameter within upper pola left kidney.

Impression:
Ill-defined enhancing lesion within posterior medial aspect of right hepatic lobe suspicious for metastasis disease. Increases in size of lesion within the infer lateral posterior right hepatic lobe with evidence of delayed enhancement suggestive of metastasis disease. A cystic area with minimal peripheral enhancement within anterior right hepatic lobe in area of previously noted metastasis lesion suggestive of post treatment change. No definite residue/recurrent tumor at the location is suspected.
